RULES: |

(1) Considerations must be submitted on 1/2 inch VHS video cassette or DVD. To be eligible, your work must
have aired on television or been released theatrically between January 1, 2011 and December 31, 2011.
(2) Deadline for submission is: February 15, 2012.
(3) If you wish to submit more than one performance on the same VHS tape or DVD, be sure to label the tape/DVD
in the correct order. However, each separate performance must be submitted on a separate Nomination Entry Form.
(4) Please include a photo and resume with your Nomination Entry Form.
(5) All tapes, DVD's, photos and resumes become the property of the Young Artist Awards and will not be returned.
(6) Mail your Nomination Entry Form, photos, resumes and VHS tapes/DVD to:
